Dr Cassileth has been involved with the psychosocial aspects of cancer care for more than 25 years and also founded the Society for Integrative Oncology, which publishes a medical journal. She has just released a new book, The Complete Guide to Complementary Therapies in Cancer Care. MSKCC’s Integrative Medicine Service’s Web site, www.MSKCC.org offers evidence-based information about herbs, vitamins, and unproved cancer treatments at no charge to professionals and the public.

The Service’s research includes studies to evaluate the ability of specific complementary therapies to reduce important symptoms associated with cancer treatments and the investigation of botanicals for potential antitumor effects.

Spontaneous Remission launched Caryle Hirshberg into the cancer spotlight when it was published in 1993 by the institute of Noetic Sciences (IONS). The book filled a gap in research, as before that time there was no standard reference for the field of spontaneous remission. Then, in 1994 Remarkable Recovery was published; a book of stories about patient unexplained recoveries. Both  book looked at unexplained recoveries from medical literature and although considered a rare phenomena – they were surprised at the number of references they found. It piqued their interest to know more….

Just for a moment, turn back the clock to 1994. Can you imagine the New York Literary Guild’s Lawrence Van Gelder writing in the NY Times that the three hottest books on the horizon were ‘Like Water for Chocolate’, ‘The Horse Whisperer’ and, amazingly a nonfiction work; ‘Remarkable Recovery’ a book that offered scientific evidence that people recover from seemingly terminal illnesses more frequently than is supposed. Hirshberg’s well researched works of science not only captivated cancer patients worldwide but they marked a beginning of an almost cult-like fascination with healing in the public arena.

Caryle Hirshberg, Project Manager at IONS and the late Brendan O’Regan, Vice President of Research at IONS joined forces to create a project that had never been researched before. So was born ‘The Remission Project’ at IONS, California. The Spontaneous remission Hirshbergtask was enormous; to catalogue the world’s medical literature on the subject. This resulted in an assembly of the largest database of medically reported cases of spontaneous remission in the world, with more than 3,500 references from more than 800 journals in 20 different languages.

Caryle Hirshberg will discuss her role in this formative period in American healing history with Grace Gawler. The theme is why people heal from diseases that were deemed incurable. This was a courageous topic to pursue; to study the seemingly miraculous while keeping science and well researched data on track. This was the first time that a research group not only gathered medical data from journals worldwide; but they dared to look at why people recovered and what we might learn from their turnaround to stay well. The book was controversial, inspiring and as well gained attack from some medical corners; yet the material they researched was from medical journals. Like TV’s past science hero, Professor Julius Sumner Miller; Hirshberg and O’Regan were brave enough to ask the fundamental scientific question; ‘Why is it so?’

Cancer and the Global Equity Divide: A Call for Action.

This year more than half of the nearly 13 million new cancer cases diagnosed worldwide and two-thirds of cancer deaths will occur in the world’s low and middle income countries (LMICs). Nearly a third of these deaths could have been prevented with the knowledge and technology already available today. For example, only 10% of children diagnosed with leukemia in the 25 poorest countries of the world will survive compared to 90% of children diagnosed with leukemia in Canada.

 

The disease burden in developing nations is growing. Caused by an inequity in health, healthcare and resulting disease, the disparities across the cancer care continuum found between rich and poor countries remain largely unaddressed. The cancer divide is the result of these disparities — explained in the report of the Global Task Force on Expanded Access to Cancer Care and Control in Developing Countries-GTF.CCC and in the book Closing the Cancer Divide: An Equity Imperative.

Evidence of this growing burden in LMICs is only beginning to be translated into effective and practical solutions. Traditional rhetoric argues that the challenge of addressing cancer in poor countries is unnecessary, unaffordable, unrealistic, and detracts resources from other more pressing development programs. However, the impending cancer crisis in LMICs remains too large to be ignored. Surviving cancer at all costs often dominates the world of a cancer patient. In the alternative medicine movement in particular; blogs and websites – little if any attention is given to the “failure” factor – not popular on these sites for obvious reasons. Just imagine someone trying to sell you a “Cure for Cancer” – potions, tea, supplements and dietary regimens; if there was an “out-clause” – errr if this doesn’t work for you…here is what to do!!! It somewhat destroys the ‘faith factor’ doesn’t it.

Cancer counsellors, cancer entrepreneurs, cancer authors must be more responsible and address the issue of what to do, what strategies to employ just incase things don’t work out as hoped for – the possibility or probability of not making it through cancer or some other life threatening challenges is real for many people. Denial and avoidance are not useful ways of dealing with a truth that will – at sometime confront. Let’s face it we are all going to go at some time! Most of the people who have died in the past 12 months that I know have not been cancer patients! There have been heart attacks, accidents and other unexpected life events. So Death – the great adventure is something we all need to be prepared for….as many have said – dealing with death fees up more energy for living. This is especially important if you have been diagnosed with a life challenging illness.

This week on Navigating the Cancer Maze I interviewed Rev Dr Ian Mavor and Deirdre Hanna from Hopewell Centre on Australia’s Gold Coast. http://www.hopewell.org.au/ http://www.paradisekids.org.au/ I chose to interview this couple because they do not give lips service -they know what it is like to work at the coalface of life’s big issues ….end of life experience, grief and loss with adults and children. Their work is important and they reach many – but their concept could reach further into our society. Of course their work goes far beyond cancer. How we all deal with grief, loss, shock, betrayals etc can form the substrate of who we present to the world. I believe, and many would agree, that the way we learn to deal with life’s big issues can significantly affect health in all manner of ways.

Today on Navigating the Cancer Maze we are going to talk about one of the most if not THE most difficult issues for discussion for cancer patients, families and friends of cancer patients. This an episode to bookmark for if and ever you are faced with helping someone with an end of life experience. Talking about palliative care, grief, loss and death is often seen as an unpalatable subject for cancer patients who want to be pro active with searching for the “cure”. But the reality is not everyone makes it through the journey of cancer and one thing is certain; we all must die at some time. What can be done to prepare? How can dealing with death free up energy for living? Rev Dr Ian Mavor and his wife Deirdre Hanna have chosen one of the most challenging vocations. A survivor of uterine cancer; Fran Drescher brings a new beacon of hope; a message to all women to trust their intuition when they ‘know’ that something is desperately wrong with their health when a health professional insists they are OK.

This is Fran Drescher’s story and it can happen to any of us. Fran Dresher was Fran van helping womenmisdiagnosed  &  mistreated for a peri-menopausal condition she did not have. Her doctors told her she was experiencing symptoms because of a long list of reasons – she was too young, too thin, and another reason given was that she ate too much spinach! Fran was prescribed hormones to treat the symptoms, but her doctors made one fundamental error – they did not order proper diagnostic tests. Fran says; at the time, I didn’t know to ask why or why not, because I was just happy to be told I was too young for any other condition! Eventually an endometrial biopsy confirmed Fran’s greatest fear; she had cancer.

It had taken two years and eight doctors before finally being told that she had a gynecologic cancer. Fran says: “I felt betrayed by not only by my own body, but the medical community.”

In 2002, Fran wrote Cancer Schmancer, to tell her story of survival so what happened to her wouldn’t happen to others. “After I went on my book tour” she says, I realized that what happened to me had happened to so many women like me. And so it was then I realized the book was not the end but rather the beginning of a life mission to improve women’s healthcare in America.”

Using her own experience and new found awareness, Fran created not only the book of the same name, but the Cancer Schmancer Movement and Cancer Schmancer Foundation. Grace Gawler Institute: Prof Ian Frazer viruses and cancer – why vaccines are important in cancer prevention

Viruses, bacteria abd parasites are responsible for many cancers Researchers make a rough extrapolation from their data and estimate that of the 7.5 million deaths from cancer in 2008, 1.5 million, or about one in five, were caused by an infection. That’s a lot of deaths from preventable causes! They argue for more work on getting existing vaccines to the populations that need them and continuing research and education on vaccines in places where they’re readily available.

Last Friday I had the pleasure of listening to Professor Ian Frazer speak about his research on viruses and their implications in cancer development. This is an issue that I have been researching for some time. The lecture was both well presented, profound and inspiring. The more I have read the research about viruses and cancer, the more interested I became in education and awareness programs in prevention of virally induced cancers. The Grace Gawler Institute’s association with Dr Ursula Jacob Hallwang private Oncology Clinic and RGCC Greece – a world-class laboratory which specialises in medical genetics and in particular cancer genetics; marks a signpost for the future of genetics  in both prevention and treatment. While many people are pursuing extreme dietary measures to eradicate their cancers; most do not know about the role that viruses have played in the development of their cancers. Our associates are working diligently on treatments to nullify the effects of viruses once a cancer has already been created; however Professor Ian Frazer and colleagues are working towards prevention and eradication of cancers that are known to be virally charged… and their solution is vaccination of young people in the high risk groups. New Vaccination Program: Boys aged 12 – 13 will receive the vaccine through school-based programs under the National Immunisation Program, with Year 9 boys also included in a two-year catch-up plan.

The HPV vaccine has already contributed to a decrease in pre-cancerous cervical lesions in young women.

When administered to males, it will help prevent cancers of the genital tract, some types of head and neck cancers, and it will also enhance the vaccine’s effectiveness in women. With sexual activity occurring at earlier ages in combination with the types of sexual practises promoted today, these sexually transmitted viruses acquired in teenage years are responsible for many cancers in the 30- 40’s age bracket. HPV is also implicated in anal cancers and now implicated in some skin cancers.

Anti vaccination groups or individuals will likely be against the latest government objective when most Young Australian males will receive the HPV vaccine (Gardasil) free of charge in a world-first public health measure that will help prevent a range of cancers. Professor Frazer addressed this issue saying “There’s a very small group of people out there, who argue that we shouldn’t use vaccines, and they influence a much larger group of people not to get their children properly immunised,” he said. He has been involved in making a documentary that will provide a counter argument to the small minority that vaccines are safe and wonderful.”

Dr Frazer spoke about Bhutan which has one of the highest incidences of cervical cancer per head of population in the world. He estimates that Bhutan would take about 15-20 years from today to see a decrease in cervical cancer, he said, the time it takes between the virus infection and when one gets cancer. The girls that Bhutan is immunising today, at 12, he said, would be at the “maximum risk of cervical cancer in their 30s and 40s” and, by then by, there should be “virtually no cervical cancer in those girls.” Recommended extra reading:

German Cancer Treatments and the Grace Gawler Institute – hyperthermia and other therapies

During our time in Germany (March-April) as guests of Dr Ursual Jacob’s clinic, we were introduced to many outstanding cancer specialists. Many patients travel to Germany hoping that there is an alternative medicine cure for their cancer and many are disappointed to discover that it is the conventional medicine offered in Germany that forms the majority of the treatment plan. Complementary adjunct treatments are used to assist the body to tolerate some treatments and while helping to build resistance to recurrence of cancer.  However just as we have innovators in medicine here in Australia, so too does Germany and from my personal experience; the Netherlands is out there in innovative medical techniques. Where these innovators differ from our conventional medicine is that chemotherapy is often delivered to a patient via different way. The German Clinics have many different techniques to obliterate and control tumours; here are some of them.

For the purpose of this blog I will focus on liver cancer; secondary liver from primary bowel cancer primary liver cancer and breast cancer that has spread to the liver. Because the outcomes of liver cancer are often poor the public perception is that there is little that can be done. This is likely why so many of the liver cancer patients we see; have taken the route of alternative medicine, meditation and dietary approaches; abandoning conventional medicine. Patients will often say they feel well while practising these lifestyle approaches; but they come to see me because their tumours continue to grow and they need a solution….quickly.

So – here are a few of the German Cancer Treatments that are being effectively and successfully used for liver cancer.

At the University of Frankfurt, we are introduced to Prof Vogl. He welcomes us to his section of the university then it’s down to business. Everything about his demeanour gives an air of precision, excellence, efficiency and when we see him performing his crafts – Chemoperfusion and chemoembolisation – we are suitably impressed!

Gowned up in lead aprons we film the day procedures. We watch on a screen as the chemo is delivered directly in to the tumour as Chemoperfusion is performed via entry into the femoral artery of the patient with a special catheter guided to the liver. We are told that application of chemotherapy through the major arteries into the liver arteries allows one to achieve a concentration 100 times higher than by systemic approach with only minimal adverse effects.

TACE Trans aterial chemoembolisation German Cancer TreatmentsTransarterial Chemoembolisation TACE Embolising (blocks) the branches of the hepatic artery which supply the tumour causing starvation of blood supply and death of the tumour while the normal liver tissue will retain its blood supply through the portal vein. The washout of the concentrated chemotherapy is also delayed when using this method. The half-life of chemotherapeutic agent is increased by hours to weeks through the stoppage of blood supply.

In TACE, the knowledge of difference in blood supply to the tumour and liver tissue is used. Up to 75% of the normal liver tissue is perfused by the portal venous system and only 25% is supplied by arteries. On the contrary, liver tumours are supplied up to 95% by arteries. Hence chemoembolization of liver arteries lead to development of ischemic necrosis in the tumour region while the remaining normal liver tissue is spared by sufficient perfusion through the portal venous system.

LITT – Laser induced interstitial thermotherapy.

Development of LITT

The present form of laser therapy has been developed by Prof. Vogl and Prof. Dr. Mack in close co-operation with Dr.Roggan from Laser and Medicine Technology GmbH (LMTB), Berlin, Germany. The procedure is continuouslyoptimised and used routinely in clinical practice with greatsuccess.

How it works: In practise a temperature of about 60 to 110° C is achieved in the tumour tissue. This differentiates LITT from the classical hyperthermia.

The use of local thermal effect in the tumour region forms the basis for this new minimal invasive therapeutic procedure. The energy of laser light is absorbed, which causes heating of the tissue. The heat causes coagulation (destruction) of tumour tissue and the edge around it. LITT can be effectively used in may cases of unresectable cancer (by surgery) or for patients who failed a liver resection.

According to the professor, it is scientifically proven that local resection or destruction of liver metastases prolongs the life of patients and chemotherapy is then to be preferred.

LITT can also be tried as an alternative therapy for patients who refuse surgical resection and systemic or local chemotherapy.

Turning up the Heat on Cancer Cells:
Channel 7 Sunrise created a groundswell of interest around Australia this week with their telecast of a segment about hyperthermia in Germany.

In this third blog on this subject, I would like to discuss the importance of hyperthermia in a treatment program for cancer. Parmenides, a Greek physician and philosopher (540-480 B.C.), said “Give me a chance to create a fever and I will cure any disease.” In 2011 that sweeping statement can be qualified more accurately as to how much hyperthermia can help bring about a remission from cancer.

In the past 2 years many clients have arrived at our Institute with a poor prognosis – many having exclusively used alternative medicine for years despite the fact their cancer would have been treatable by local conventional medicine had they taken that path.  Some of these patients have been able to gain life quality and extend their lives up to 2 years after visiting German clinics who have offered among many therapies – hyperthermia.   A rare few in this group have worked hard for their remissions & their survival given tumour load has been remarkable. The Institute has supported many cancer patients in their quest for recovery and it is a great feeling to know that many of our clients who had dire prognoses, are spending another precious Christmas with family & friends. However I also remember each Christmas those brave souls who didn’t make it despite the best treatments here and overseas. Working with cancer is a sobering experience.

The moral of the cancer story is clear – find it early – treat it early & don’t waste time playing Russian Roulette with unproven internet & alt/med “cures”.

Local Surface Hyperthermia –In addition to the treatment with Whole-Body-Hyperthermia (WBO) and Local Hyperthermia; patient’s can be given Local Surface Hyperthermia. This therapy method is especially suited for superficial tumors such as skin cancers, superficial lymph nodes and metastases of the skin and / or the muscles of different primary tumors (e.g.Malignant melanoma, superficial metastases of the Breast).

Local Surface Hyperthermia

For the Local Surface Hyperthermia the heat is generated with a water-filtered infrared radiation source (infrared A) which is able to penetrate into the tissue to a depth of around one inch (up to 2 cm). The whole procedure takes one hour.  Often Local Surface Hyperthermia is given  in combination with chemotherapy or immune-therapy to increase the effectiveness of the therapy.
The Local Surface Hyperthermia is a safe, non-invasive therapy without harmful side effects.

2. Local Hyperthermia For a long time it has been known that cancer cells may be damaged by heat.  If you heat up cancer tissues, heat shock proteins develop, initiating immunological mechanisms of defence against cancer cells.  Repair mechanisms in the cancer tissue after chemotherapy or irradiation get impaired by hyperthermia.  If the chemotherapy or irradiation is performed incombination with hyperthermia these treatments will be much more effective.

Local Hyperthermia

As a result, in many cases it is possible to use a lower dosage of chemotherapy which means less toxicity for the patient.

In local hyperthermia, cancer tissue or metastases are heated up by using short wave irradiation with 13.56 Mhz and an energy up to 150 watts.  Penetration depth is about 20 cm.  In the tumour tissue, temperatures higher than 42 c (107.6 f) are achieved.  While healthy cells tolerate this treatment malignant cells get damaged.  The local hyperthermia has no significant side effects.  During the treatment the patient stays relaxed on a warm water bed.

 3. Whole Body Hyperthermia (WBH)   The healing effect of fever has been known for many centuries.  Whole body hyperthermia (WBH) is a new treatment using the well known principle of fever in a therapeutic way. There are different ways of action: • Fever to activate the immune system • Heat as thermic damage of cancer tissue • The thermic increase of the efficiency of chemotherapy

  3 a/ Moderate Whole Body Hyperthermia– stimulates & activates the immune system.  This treatment is given when chemotherapy is not appropriate.

Whole Body Hyperthermia

The body core temperature is raised to about 39.5 c (103.1 f), which simulates a natural fever increasing the number and activity of natural cells, T-helper cells and cytotoxic T-cells.  This treatment is also used in cancer diseases with special association to the immune system like renal-cell-carcinoma, malignant melanoma and special lymphomas. Moderate whole body hyperthermia is also used to prevent recurrences.

 3 b/ Extreme Whole Body Hyperthermia – particularly in advanced or metastatic disease. Extreme whole body hyperthermia is used in combination with chemotherapy in advanced or metastatic cancer.  The body core temperature is increased up to 42 c (107.6 f).  Extreme whole body hyperthermia is useful in advanced cancer, especially with metastases in different organs, e.g. in the liver, bones or lungs. Together with whole body hyperthermia, chemotherapy is more effective.  We start the chemotherapy at a temperature of about 41 c (105.8 f).  Very often it is possible to use very low doses of chemotherapy, so side effects of the chemotherapy are kept to a minimum.  Tumours or metastases resistant to chemotherapy can be successfully treated with a combination treatment of chemotherapy and whole body hyperthermia.  Body core temperature is increased carefully using whole body water filtered infra-red-A-irradiation.  The extreme whole body hyperthermia is a safe treatment. During the whole body hyperthermia the patient is in a special unit (IRATHERM 2000) and can be reached from all sides. To be continued…..
