Defining Survivorship
Surviving cancer or “survivorship” can be defined in different ways. Two common definitions include:
• Being disease-free after the completion of treatment,
• Living with, through, and beyond cancer includes people who continue to have treatments to either reduce risk of recurrence or to manage chronic disease. By this definition, cancer Survivorship begins at diagnosis and survivorship is a state that continues throughout your life.
• Bernie Siegel described this group as EXCEPTIONAL Cancer Patients.

Because we are talking about the precious lives of cancer patients – as a part of today’s subject, I hasten to add here that it is important for those interested in surviving cancer that I discuss where NOT to put your energy if you want to be a survivor. Yes modern medicine is not perfect – but it is the best we have and it it is improving all the time.

If you are seeking Survivorship through and beyond cancer – please use every means possible from the best that conventional medicine has to offer in combination with the best of scientific complementary medicine and complementary therapies.

baby-bathwaterPlease “Don’t throw the baby out with the bathwater”! Conventional medicine is a gift. Cancer is tricky – you can’t outsmart it with juices, meditation and positive thinking. How “you be” with a cancer diagnosis – i.e. – your attitude, strategies, will to live and will to heal – contribute tremendously – but you need to partner with the best that conventional medicine has to offer. If cancer patients understood the scientific basis of cancer – how it behaves and operates for it’s own survival; different choices might be made- conquering cancer is not that simple!

 I must mention Jess Ainscough here because of the influence she has had on cancer patients young and old in terms of promoting one type of Survivorship approach. Surviving through and beyond cancer is not the simple 123 steps that entrepreneurs such as Jess promoted.

Cancer entrepreneurs like Jessica Ainscough AKA The Wellness Warrior has influenced thousands, maybe millions to take the alternative path to cancer healing. Sadly it all went wrong for her mother who had breast cancer. She died last year. Jess remained struggling with advancing fungating cancer until this week when she died from her epitheloid sarcoma. Sadly I see patients like Jess every week. I conduct an Alt med cancer rescue practice with my Specialist GP colleague Dr Whelan. We see so many scales-of-justicepatients influenced to take the natural path to cancer recovery – influenced by Dr Google and the plethora of misleading books by cancer entrepreneurs.

Jess became a crusader for the Gerson diet and might I add, was also influenced by my ex-husbands ‘cancer recovery’ story. During the past few years his story has been challenged by myself and more recently; by two Melbourne oncologists who published their findings in the internal Medicine Journal. Once they discovered there was no biopsy performed for Ian’s condition at the time which was thought to be secondary bone cancer they began an independent investigation into why he recovered. In 1978, here was no other explanation offered and to all and sundry it appeared as though the impossible had been achieved – a full remission from metastatic osteogenic sarcoma. However with a thorough review and missing pieces of the Gawler healing puzzle put in place – a new diagnosis was suggested.  IMJ HAINES AND LOWENTHAL

Jess Ainscough’s approach to her cancer was clearly influenced by Ian Gawler’s recovery, the diagnosis which was highly likely to have been advanced TB with calcified abscesses – not metastatic bone cancer as had been reported over the decades. (see the menu for more on this subject).

 Importantly –  If that is so – then the basic premise and promise upon which Jess Aincough based her recovery from cancer plan was false. Jess was quoted as saying: ” Ian Gawler was diagnosed with bone cancer and had his right leg was amputated in 1975. However, the disease recurred later that year and began ravaging his body. Ian’s story of recovery, employing an integrated approach driven predominantly by dedicated meditation, is truly remarkable. It was my anchor to a future the doctors had pretty much ruled me out of having. I thought, If Ian can do it, so can I. His book, You Can Conquer Cancer, was my Bible”. Clearly patients like Jess are looking for options and they will often choose the seemingly softer options that the one size fits all alternative approach offers. Cancer education is imperative and sadly lacking in the patient world. Cancer is not a generic disease that can be simplistically tackled with food and copious juices. I think this is a very sad indictment of what true Survivorship is really all about. Like many of the patients I see in my practice, Jess Ainscough only embraced conventional medicine when the path she had followed failed her.
It is my hope that Jess Ainscough’s story and life serves as a teaching story for all who want to walk the path to cancer Survivorship – please use critical thinking to assess information on the internet and consider a best of both worlds approach for best outcome.

Listen to Your Body Clock – Cancer and Your Health – Grace Gawler

We have knowledge of many cycles in our world. The tides, day and night are the most obvious. All creatures great and small are subject to these unseen forces that ultimately shape our life and our experience of it.” The circadian clock in mammals drives many physiological processes including the daily rhythms of sleep–wake behaviour, hormonal secretion, and metabolism. Less known, is that the clock also drives our immune cycles; an important factor in cancer treatment and recovery says Melbourne University Research Fellow Martin Ashdown, who has studied the phenomena for a long time. The science of taking advantage of the body’s own immune rhythm to improve efficacy of treatments is worth paying attention to whether its surgery, chemotherapy, immunotherapies, radiation/cyberknife; one thing is now clear – timing of treatment matters!

Research work of Synchronizing cancer treatment with the patient’s immune cycle has been recently launched into our awareness by Martin Ashdown and Brendon Coventry – and hopefully this blog, and my radio show Navigating the Cancer Maze on Voice America’s Health and Wellness Channel. Please spread the news!

With regards to timing – Did you know that majority of best-selling drugs in the United States target circadian gene products? Consider the case of statins, a class of drug that lowers cholesterol by inhibiting HMGCR (HMG-CoA reductase). HMGCR is the rate-limiting enzyme in cholesterol biosynthesis and its activity peaks during the night. Statins with short half lives showed maximal efficacy when taken in the evening (when their target gene was most active).This is just one example of chronotherapeutic practices positively impacting drug treatment. In other words – it’s all in the timing. (see resources below for more information on this).

In terms of Circadian rhythms and body clocks – simply reconsider an old message for good health. Early to bed and early to rise makes a man healthy, wealthy and wise!”

While an association of chrono-disruption was initially proposed only for breast cancer, we recently suggested a more general theory, i.e., chrono-disruption may aggravate the development of many cancer types.”

“Part of their conclusion is that “Circadian rhythmicity is an integral component of normal and optimal physiology. Not only the SCN, but cells throughout the organism are equipped with internal clocks that control the molecular events that occur during each 24-hour period. These events appear to be precisely timed and in modern societies the intrinsic time keepers are provided with misinformation which, when prolonged, likely has ramifications in terms of increased pathologies.

It would seem judicious to take better care of the circadian network by providing appropriate photoperiodic cues, which currently seemed to be almost totally ignored. In addition, large amounts of this indoleamine (melatonin) is produced in the mucosa of GIT that seems to serve as local antioxidant and protective substance for the gut and liver against a variety of noxious agents, particularly the bacteria and their toxins, introduced into the gut with each meal.

Measuring immune cycles
Measuring immune cycles may be one missing piece of the cancer cure puzzle!

Two cancer researchers Martin Ashdown & Brendon Coventry have explored that question and yes they are excited! So am I and so are some doctors and oncologists! Testing patients to map their immune cycle is not a complex exercise, but to do this, a system needs to be changed. All discoveries of worth seem to be hobbled for while – then accepted – struggling across the finish line…sometime after decades!

In 1983, Australian doctors J. Robin Warren and Barry Marshall isolated Helicobacter pylori, the bacterial cause of peptic ulcer disease. When the H. pylori hypothesis was developed, the doctors were certain that it would immediately gain universal acceptance and that within two years peptic ulcer therapy would be essentially an antimicrobial regimen”. This was far from the case. A 1986 book on Peptic Ulcer and its Drug Causation lends a page to the bacterial hypothesis, noting, “Whether these bacteria have an aetiological role in peptic ulcer gastritis is unknown”. At this time, doctors still prescribed drugs to reduce stomach acidity and monitored patients for complications. There are ethical laws in place; essential for the safety of humans in medicine. Changed laws of medical ethics, prevent a modern reprise of Pasteur’s heroic demonstration of his rabies vaccine through the vaccination of an infected boy. Measuring each cancer patients immune cycle and timing their treatment at the right time –  is nothing akin to that! It simply makes sense!

Perhaps one can understand the caution in prescribing an antibiotic. Measuring immune cycles and timing treatment for a better outcome? Now to my way of thinking this fulfills the old Hippocratic oath – “Above all, do no harm!”

Today’s Navigating the cancer Maze will give insights With 30 years experience as a botanical medicine practitioner, I know that complementary medicines are very popular with cancer patients. Some believe they are an alternative to conventional medicine, rather than a complement or adjunct to other treatments. Patients tend not disclose natural medicines they are taking when seeing their oncologist. Some oncologists don’t ask. Some patients take an each way bet and use both conventional and “natural medicines”, but in most cases; they will likely never tell their doctor what they are doing. Why? Because they expect a prejudiced attitude, either ridiculing their choice or telling them to stop everything else they are taking or doing that is not considered real medicine. Herein lies an enormous modern-day dilemma because many alternative & complementary medicines are indeed REAL medicines with active phyto-chemical contents.In today’s episode and on this blog – I also share useful information about Curcumin as a adjunct medicine for cancer patients.

 

First of all Vitamin C

I often see patients in my practice who refuse to have pharmaceutical drugs….they are often shocked when I explain to them that so called “natural medicines” they are using may not be natural at all – These substances actually become a drug or pharmaceutical product once they are administered by intravenous injection or intramuscular. It doesn’t mean they wont work – but the fact is they are NOT natural as is often marketed. They may have natural origins or have active principles of natural origin that has been synthesized in the laboratory.
Perhaps the best example here is vitamin C – there is no way we could use ascorbic acid as an i/v infusion….It must be modified in the laboratory.

  • Now this may come as a shock to those to love all things natural…..but Unfortunately, nearly all the dietary supplements marketed as vitamin C these days are synthetic isolates, made with genetically modified (GMO) corn from factories in China. Ascorbic acid is theoranges-and-lemons product most often paraded as “vitamin C,” but it’s really only a laboratory-produced synthetic “isolate”—meaning it is a single component of vitamin C. Mother Nature knew what she was doing when she created vitamin C-rich fruits, berries and plants. Scientists now know that vitamin C isolates such as ascorbic acid do not provide the health benefits that whole food forms of vitamin C do.

  • Curcumin – Tumeric

Multiple Molecular Targets of Curcumin
Curcumin attacks multiple targets, providing the scientific basis for its effectiveness in many different diseases. Extensive research shows most diseases are caused by dysregulation of multiple signaling pathways–casting doubt on the effectiveness of monotherapy, which is limited to a single target.
Studies show curcumin modulates numerous molecular targets, including: regulating several cytokines and fibroblast growth factor-2 (gene expression), growth-factor receptors including modulation of androgen receptors (protein kinases), transcription factors, pro-inflammatory enzymes (including supression of COX-2, 5-LOX and iNOS and regulation of NF-κB), modulation of cell-cycle-related gene expression, blocking the adhesion molecules, downregulating anti-apoptic proteins and inhibiting multi-drug resistance.

curcumin.2jpgCurcumin is the most important active ingredient in turmeric, and makes up about 2-6% of the spice.

While therapeutic properties of turmeric have been known for centuries, modern science has identified the curcuminoids (phenolic compounds found in turmeric) and provides a scientific basis for many clinical uses of standardized curcumin.

Since turmeric contains very small concentrations of curcumin, look for standardized 95% curcumin (curcuminoids). Why? Hundreds of scientific and technical papers confirm: clinical results were obtained using curcumin (curcuminoids).Most Curcumin Products Contain 3 Curcuminoids
Commercial curcumin typically contains curcumin I (~77%), curcumin II (~17%) and curcumin III (~3%) as its major components.
For convenience, all curcuminoids are often referred to simply as “curcumin” even though turmeric contains a variety of different curcuminoids. Unfortunately, pure curcumin (including all known curcuminoids) is very poorly absorbed into your bloodstream after oral ingestion due to rapid metabolism in the liver and intestinal wall, and rapid systemic elimination.

Many curcumin products add piperine to improve absorption of curcumin. But piperine is a problem for many consumers because the additive should be taken cautiously (if at all) by anyone taking medications. According to the American Society for Pharmacology and Experimental curcuminTherapeutics, piperine is a potent inhibitor of drug metabolism, which means piperine spikes blood levels of many prescription medications.
A review of published research articles reveals piperine is toxic in experimental animals. Some research suggests piperine is safe in small amounts but large amounts of piperine could be damaging to the liver or other organs. Experts advise against consuming more than 15 mg of piperine per day. Piperine has known central nervous system (CNS) depressant effects. Finally, since piperine is a component of black pepper, consumers with a known allergy or hypersensitivity to black pepper should avoid piperine.

Cheap versions of complementary medicines manufactured in countries with poor manufacturing and quality control standards are best avoided…….contamination is a big issue as well as growing, harvesting and all that happens to a the material before it gets to production. A recent example in May 2013 was the outbreak of symptomatic hepatitis A virus infections across 10 US states associated with pomegranate arils imported from Turkey and manufactured in the USA by an organic group. The source was traced to imported frozen pomegranate arils. These were identified as the vehicle early in the investigation by combining epidemiology—with data from several sources—genetic analysis of patient samples, and product tracing. There were 165 cases known to have been affected.Hepatitis A is spread when human feces contaminate food or when an infected food handler prepares food without using proper hygiene. Human feces are expected as the cause of the outbreak, according to the Wall Street Journal.

Remember – it’s best to ask for professional help when selecting products from the internet: Not only is it important to know the source of raw material – but the technique used in the manufacturing process and efficacy of the end product. Sometimes the pseudo-science that surrounds a product can be misleading.
Medicines and supplements purchased on the Interne need to be carefully scrutinized, especially if you are have cancer. Countries that have raw materials processed and manufactured with inadequate standards and poor quality control can be an issue.

We all have a pelvic floor and when something goes awry with it  – it can create horrendous  physical suffering, social isolation and psychological trauma. You won’t hear this discussed very often because it is an area that suffers from the stigma of Silence! We tend to only think about pelvic  floor issues in women – but men can have just as many issues with their pelvic floor. Yes – I can’t say it too many times – I have a mission to break the silence that surrounds pelvic floor issues. If my problems had early intervention, my life would have been very different. So I figure we should all use what we have been given in life as teaching stories to educate and help others to access good information and therefore make informed choices to improve health and life! 

The interviews you will hear on navigating the cancer maze radio today were recorded live in Singapore whilst here for surgical procedure # 22! The majority of those surgeries were performed prior to my undergoing interstim treatment; first in the Netherlands with Dr Rudd Schouten and pelvic floor specialist  Marijke Sleiker ten Hove  – this was  in 2002, 2003. I was the “first” at that time for Medtronic interstim to work for my condition. It was verified that innervation to my rectum had been grossly disturbed during a routine surgery in 1997, making bowel function impossible; faecal impaction followed and  colon was removed whilst the cause – nerve damage; remained.

  Although the surgeons who performed procedures saved my life several times; for which I am grateful, I remained frustrated because the caused was continually bypassed and my issue became shrouded in Silence. I began to to study sacral nerve function and the pelvic floor. I was told it was impossible that any form of restoration existed!  The longer my problem existed the worse it got so by the time I found a possible solution with interstim; I had a lot of internal and external damage, hernias from “bags”, burns from ileostomies, further proplapses in my already damaged pelvic floor. Life quality became far too compromised Now 2 devices replaced – one in 2009  and now another in 2014  – both performed in Singapore with the surgeons I interview today. Guess what – the procedure is till off label around the world for my condition!! It is available but you need to find those skilled in this area and you need to pay for it. – no insurance. But what is a life worth I keep asking myself? In Singapore I felt understood and listened to as I had been in with my first Dutch Surgeons – when my procedure was purely an experiment. Now, to Singapore……

Dr. Lim is very passionate about improving the quality of his patients’ care. In caring for his patients, Dr. Lim has always maintained a steadfast principle, “Make decisions for your patients as if they are your loved ones. If your decision is not good enough for your loved one, it is not good enough for your patient.” For his dedication toward patient care, he has received numerous awards for excellence in patient care through the years. When I read this I knew I was in the right hands.

DEATH ADVENTURES: It would to nice to think that all cancer patients could find healing and recovery. I would like to share a recent experience that was a case of breathing life into death!  Prior to my recent visit to Germany, I had the pleasure of being invited one Saturday afternoon to a Death Cafe! I invited a guest and her family to attend with me. They were about to embark upon her journey into death. I will call Death Cafe navigating the cancer mazethis patient Lois. Lois had a highly aggressive brain tumor stage 4 glioblastoma multiforme – she was also a survivor of breast cancer 10 years prior. Following surgery and a recurrence soon after; Lois had successful treatment in Germany. However – she was not the same person that she and others knew, and this was tremendous source of distress to her. Like many patients with a brain tumor, Lois suffered from depersonalization (a state in which one’s thoughts and feelings seem unreal or not to belong to oneself). Medications and therapy did not assist. An intense episode of Shingles that lasted for 6 months followed by seizures and blindness in one eye and then a recurrence of the tumor, finally ‘tipped the scales’……Lois decided enough was enough!  She decided that she wanted to end the suffering and refused further treatment. Lois and her family gained a tremendous amount of information by attending a Death Cafe. Soon she was a patient at Hopewell Hospice and a few weeks later – she died peacefully at the Hospice surrounded by love with friends and family in a conducive atmosphere of candlelight and peace – one of the most well prepared death experiences that I have been privy to. There was openness and disclosure. Nothing was left unsaid nor undone as Lois took her final breath.

The mere mention of a Death Cafe is a definite conversation stopper! This is especially so when friends ask “What are you doing this weekend” and you reply you attending a Death Cafe function! Immediately people imagine a group of people resembling Abby from NCIS!  However – everyone who attended the Death Cafe dressed quite normally and there was not a ‘Goth fashion’ in sight! The format was a discussion by grief counselor Beth O’Brien and funeral director Neil Davis. They spoke openly about all the things death cafeyou likely thought that you would never want to know about death and yet; these are things that one should know. And  – with the room to capacity – it was amazing how the conversation became lively, questioning and engaging. People really wanted to ask more and more questions ….questions that are indeed relevant to us all.

Death is often ‘the elephant in the room that no one wants to discuss’. However it was good to see the topic on the table for discussion on ABC TV ( Australia) last night – so perhaps the Death Cafe is in deed making an impact in bringing death out of the closet.

What is The Death Cafe? It is a global movement where people come together in a relaxed and safe setting to discuss death. It’s not all doom and gloom. The organizers say that sometimes people need to understand death so they can live life to the full.
