I am honoured to be one of the 5 women they featured and it goes without saying that a Dutch surgeon, Dr Rudd Schouten ( 2002) and 2 Singaporean surgeons, Dr Francis Seow Choen and Dr Lim Jit Fong (2009), from Fortis Surgical Hospital, Singapore, were also major players. A team effort for survival. I am forever grateful to them for humanity & surgical skill.

You will read how life sometimes throws us a curve ball…… just when everything appears to be going smoothly! Mine came in 1997 when my 21 marriage ended suddenly. Three days later, another blow; a total prolapse of my uterus changed my life direction even more. A surgical procedure aimed at repair and health restoration saw the unthinkable happen – Complications left me without bowel function. As you can imagine if you have ever been constipated for a few days…..the consequences were horrendous and over time, large sections of impacted colon had to be surgically removed.

This part of my story has also been recorded in my Memoirs Grace Grit and Gratitude.( pub 2008) Copies available eBook, soft cover or Click here to read on Google books. This is a free read. Section 39 talks about my Bionic surgery.

I hope that the inclusion of my survival story in Global Health and Travel magazine, inspires others to take a chance and walk in the field of all possibilities. On this week’s Navigating the Cancer Maze on Voice America radio, I presented an encore broadcast of a special interview with my colleague GP Dr Bruce Whelan: “Spinning Gold from Straw: How trauma transformed a Doctor’s Life and Practice”. This year, Dr Whelan and I have forged a very special medical partnership to help patients Navigate the Cancer Maze – Health Intelligence Australia.

This year, Dr Whelan and I have forged a very special medical partnership to help patients Navigate the Cancer Maze – Health Intelligence Australia.

Last week a patient asked me what makes Dr. Bruce Whelan unique in terms of the type of medicine he practises? The answer came

easily. In the past 10 years Bruce has become involved with and specialized in management of addictions, mental health and severe and chronic pain as well as general medicine. These are also all important aspects of cancer management. They are the areas of medicine that tend to be under-serviced by GPs due to time constraints or lack of training. But there is more that makes Dr Bruce Whelan uniquely suited to the new Health Intelligence Australia group.

Another area of Dr Bruce Whelan’s expertise is in the area of trauma. He knows a lot about trauma at a very personal level because in 2002 he was summoned to Bali from Sri Lanka, where he was working. He was given the unspeakably grim task of identifying bodies after the bombing at the Sari Club in Bali. His task was made even worse when he spent two days unsuccessfully trying to find and identify the body of his son-in-law Shane – killed in the bombing along with a partner of his daughter’s friend. As luck had it the girls were going to be late getting to the Sari Club that night. But lucky as they were to escape; the trauma of the incident lived on and shaped their future.

When Bruce graduated from the University of Queensland in 1967, like many of us, he had no idea what life had in store for him. As a doctor, Bruce Whelan had already seen lot of suffering, but the post-traumatic stress affect that the Bali incident produced, had long lasting effects. The product of uninvited change! Bruce fought a battle he could not win; to have bodies quickly identified, stored appropriately and repatriated to Australia.

This intensified the trauma, grief and loss experienced by many. However, being a resourceful doctor, eventually he was able to spin gold from straw as they say, and today, 14 years later; the experience brings with it a man who is the wounded healer who serves his patients with a rare quality of directness and authenticity. He is someone who deeply understands trauma and depression in another human being.

There is no substitute for this type of life experience. This type of experience can’t be learned in any university except in the university of life!

It is recognized by psycho-oncologists that a diagnosis of cancer is also a traumatic event and many patients experience PTSD (post traumatic stress disorder) that often remains undiagnosed and untreated throughout the course of the illness. Some patients have experienced trauma before diagnosis and it may not have been dealt with. Unrecognized trauma can play a complicated role in the ongoing management of illness. Major life traumas can impact every treatment approach that a cancer patient will try during their recovery….. it can even affect patients who are survivors of the physical illness, but who did not have the opportunity to deal with the psychological impact of the illness at the time. This often plays out as depression, memory loss or dissociative disorders.

Death | Discussing the ‘Elephant in the Living Room’ Beth O’Brien with Grace Gawler

There’s an elephant in the living room & no one mentions it; it is called death, the subject for today’s show. I work in a pro-life vocation with people with life challenging illness, but what happens when life is not prolonged & death is inevitable? There are not only physical deaths that are relevant to today’s topic. There many “deaths” throughout life – the losses that occur as a a part of being human; loss of youth, loss of job, loss of identity, to name a few. But there are also overwhelming “deaths” throughout life – life’s big uninvited events that are felt so deeply that the grief seems endless and indeed may endure for life. These events are a apart of being human and shape the way we be on this planet. The person grieving a physical death will often get more support than the person grieving a divorce experiencing hidden pain or loss of a body part hidden from view by clothing. But grief, loss bereavement exists and it exists in many forms. Many patients have said to me that when their partner left them it felt worse than a death – that death was final – but it was challenging to accept the finality of an event when there was no physical death and the knowledge that every family event in future will have a “emotional stamp” on it!

Beth is a Certified Thanatologist; Certified in Thanatology: Death, Dying and Bereavement”, an international accreditation with the Association for Death Education and Counseling. She is also a qualified Facilitator of Death Café, an international movement designed to increase awareness of death with a view to helping people make the most of their (finite) lives. Today’s discussion will be a valuable resource, bringing death to the discussion table.

After 20 years studying, researching and working with people who were experiencing loss and grief, Beth O’Brien discovered a strong connection to work with people who were experiencing grief and loss.

Recently she became a Certified Thanatologist, Certified in Thanatology: Death, Dying and Bereavement”; an international accreditation with The Association for Death Education and Counseling. In her work as a marriage and funeral celebrant she feels privileged to work with people on the most important and memorable days of their lives. Beth became so enthralled by the stories and eulogies she heard, she decided to become a personal historian and is a member of The Association of Personal Historians. DEATH ADVENTURES: It would to nice to think that all cancer patients could find healing and recovery. I would like to share a recent experience that was a case of breathing life into death! Prior to my recent visit to Germany, I had the pleasure of being invited one Saturday afternoon to a Death Cafe! I invited a guest and her family to attend with me. They were about to embark upon her journey into death. I will call this patient Lois. Lois had a highly aggressive brain tumor stage 4 glioblastoma multiforme – she was also a survivor of breast cancer 10 years prior. Following surgery and a recurrence soon after; Lois had successful treatment in Germany. However – she was not the same person that she and others knew, and this was tremendous source of distress to her. Like many patients with a brain tumor, Lois suffered from depersonalization (a state in which one’s thoughts and feelings seem unreal or not to belong to oneself). Medications and therapy did not assist. An intense episode of Shingles that lasted for 6 months followed by seizures and blindness in one eye and then a recurrence of the tumor, finally ‘tipped the scales’……Lois decided enough was enough! She decided that she wanted to end the suffering and refused further treatment. Lois and her family gained a tremendous amount of information by attending a Death Cafe. Soon she was a patient at Hopewell Hospice and a few weeks later – she died peacefully at the Hospice surrounded by love with friends and family in a conducive atmosphere of candlelight and peace – one of the most well prepared death experiences that I have been privy to. There was openness and disclosure. Nothing was left unsaid nor undone as Lois took her final breath.

The mere mention of a Death Cafe is a definite conversation stopper! This is especially so when friends ask “What are you doing this weekend” and you reply you attending a Death Cafe function! Immediately people imagine a group of people resembling Abby from NCIS! However – everyone who attended the Death Cafe dressed quite normally and there was not a ‘Goth fashion’ in sight! The format was a discussion by grief counselor Beth O’Brien and funeral director Neil Davis. They spoke openly about all the things you likely thought that you would never want to know about death and yet; these are things that one should know. And – with the room to capacity – it was amazing how the conversation became lively, questioning and engaging. People really wanted to ask more and more questions ….questions that are indeed relevant to us all.

Death is often ‘the elephant in the room that no one wants to discuss’. However it was good to see the topic on the table for discussion on ABC TV ( Australia) last night – so perhaps the Death Cafe is in deed making an impact in bringing death out of the closet.

What is The Death Cafe? It is a global movement where people come together in a relaxed and safe setting to discuss death. It’s not all doom and gloom. The organizers say that sometimes people need to understand death so they can live life to the full.

Professor Mark Smyth

Professor Mark Smyth received his PhD from the University of Melbourne in 1988 & trained at the National Cancer Institute (USA) 1988-1992.

After 8 years at Austin Research Institute Melbourne, working on mechanisms of lymphocyte-mediated cytotoxicity, he relocated to Peter Macallum where his studies on effector molecules collectively rekindled world-wide interest in cancer immune surveillance. A number of his discoveries have led to new clinical trials in cancer immunotherapy.

Professor Smyth relocated to QIMR Berghofer, Brisbane (2013) as a Senior Scientist and NH&MRC Australia Fellow. He is a Senior Editor at Cancer Research, and a member of the Scientific Advisory Board of the Cancer Research Institute (USA).

Summary

The Immunology in Cancer and Infection Laboratory currently focuses upon advancing our understanding of the basic principles underlying an immune response to cancer and infection.

We aim to further understand these processes at the molecular level, with particular emphasis on the role of the innate immune system, in particular, NK cells, NKT cells and gamma-delta T cells.

Our laboratory is building a detailed picture of how networks of immune cells function to recognise, respond to, and destroy tumour cell masses and metastases. We are interested in defining the importance, timing, and nature of the natural immune response to transformation.

We are also using new antigens (including glycolipids), antibodies, and cytokines in combination to stimulate strong innate and lasting adaptive immunity to cancer.

Our findings are being used to develop more effective biological and cellular therapies for human cancer, in particular, melanoma, breast and prostate cancer, and haematological cancers.

One of QIMR Berghofer’s three Research Programs is cancer, one of the major causes of illness and death in Australia and the developed world. In 2012, it is estimated that more than 120,700 Australians were diagnosed with cancer (excluding non-melanoma skin cancer). Cancer accounted for about 3 in 10 deaths in Australia, making it the second most common cause of death, exceeded only by cardiovascular diseases. Cancer is a disease which is caused by abnormal cell growth and eventually spreads to other parts of the body. Some cancers are common within a family history and are clearly inherited, while others are caused by factors in the environment interacting with genetic susceptibilities. Many forms of cancer can be treated successfully if detected early.

Immunotherapy is not new. It is interesting to take a look at the History of Immunotherapy and it’s pioneers.

In the 1850s, doctors in Germany noticed that patients’ tumours would occasionally shrink if their tumour became infected. This observation led to the idea that the body’s immune system could be harnessed and made to fight cancer.

Around the same time, doctors throughout Europe, encouraged by the success of Edward Jenner’s smallpox vaccine, attempted to make a ‘cancer vaccine’ by injecting patients with crude extracts of tumours from other cancer patients. These treatments were largely ineffective, but the field of ‘immunotherapy’ was born.

Initial progress on immunotherapy was slow, and over a hundred years’ work in the laboratory yielded little success in actual cancer treatment. This all changed when in 1975, Georges Köhler and César Milstein, working in Cambridge, discovered how to make synthetic antibodies.

Their discovery, coupled with an ever-increasing understanding of the immune system, has led to a variety of treatments and strategies that use the immune system to tackle cancer. Some, such as the antibody-based breast cancer drug Herceptin, are now used routinely to treat cancer patients.

Professor Jérôme Galon, Ph.D. whom I recently interviewed on Navigating the Cancer Maze received the William B. Coley Award in 2010. The award was established in 1975 in honor of Dr. William B. Coley, a pioneer of cancer immunotherapy, whose daughter, Helen Coley Nauts, founded Cancer Research Institute. To understand Immunotherapy – please take 7 minutes to view this excellent video:

Immunotherapy: Boosting the immune system to fight cancer

William B. Coley, in 1891, injected streptococcal organisms into a patient with inoperable cancer. He thought that the infection he produced would have the side effect of shrinking the malignant tumor. He was successful, and this was one of the first examples of immunotherapy. Over the next forty years, as head of the Bone Tumor Service at Memorial Hospital in New York, Coley injected more than 1000 cancer patients with bacteria or bacterial products. These products became known as Coley’s Toxins. He and other doctors who used them reported excellent results, especially in bone and soft-tissue sarcomas.

Despite his reported good results, Coley’s Toxins came under a great deal of criticism because many doctors did not believe his results. This criticism, along with the development of radiation therapy and chemotherapy, caused Coley’s Toxins to gradually disappear from use. However, the modern science of immunology has shown that Coley’s principles were correct and that some cancers are sensitive to an enhanced immune system. Because research is very active in this field, William B. Coley, a bone sarcoma surgeon, deserves the title “Father of Immunotherapy.”

Further acceptance of his ideas was brought about by Coley’s own children. His son Bradley (1892-1961), also an orthopaedic surgeon, succeeded him as the head of the Bone Tumor Service at Memorial Hospital. Bradley Coley’s major textbook on bone tumors was published in 1948, and while advocating surgery as the main treatment for bone sarcomas, he supported the use of Coley’s toxin as adjunctive therapy. He believed that it would be of value in preventing micro-metastasis. His daughter, Helen Coley Nauts (1907-2001), became a cancer researcher and devoted her life to the study of her father’s toxins. She tabulated every patient he treated and reviewed all his notes. She published 18 monographs and tabulated over 1000 of his cases and noticed that in 500 of these there was near-complete regression. She founded Cancer Research Institute in New York.

Immunotherapy can be local or systemic.

Local immunotherapy delivers the treatment to the affected area. For example, the BCG vaccine can be injected into the bladder to treat bladder cancer, as it causes inflammation that can cause the tumour to shrink.

Systemic therapy treats the whole body and is useful for targeting cancer that may have spread. In the 1980s, scientists at the Cancer Research UK Medical Oncology Unit at the Christie Hospital in Manchester showed that the protein interferon alpha could cause tumours to shrink in patients with low-grade lymphoma. Interferon is now used to treat several different types of cancer.

Immunotherapy can also be non-specific or targeted.

Non-specific immunotherapy works by boosting the body’s immune system in general, so that its natural cancer-killing activity is enhanced. Both of the examples of local and systemic therapies (above) are also examples of non-specific immunotherapy.

Targeted immunotherapy is designed to make the immune system specifically kill cancer cells. The following types of targeted immunotherapy are available or are in development:

Antibody-based therapies

Antibodies are proteins produced by the immune system. A type of white blood cell called a B-cell produces them in response to an infection. Normally, antibodies stick to foreign objects in the body and label them for destruction. Researchers have been trying to make antibodies that will attach themselves only to cancer cells. This can be useful in four ways.

It can stop the cancer from growing by stopping other essential ‘growth factors’ from sticking to it.
It can ‘tag’ the cancer for destruction by the immune system.
If cancer drugs or radioactive particles are attached to the antibody, it can deliver them directly to the cancer cell without harming the rest of your body.
An enzyme (a type of protein that can promote chemical reactions) can be attached to an antibody, and then given to a patient along with a chemical that can be turned into a powerful drug by the enzyme. This directs the drug to the cancer, and minimise side effects. This process is known as Antibody-directed Enzyme/Pro-drug Therapy (ADEPT).

Professor Galon’s current interests & major contributions concern basic & translational research in cancer immunology, using systems biology.

His laboratory has made some ground-breaking findings demonstrating that the adaptive immune response within a tumor was a better predictor of survival than traditional staging based on the size and spread of a tumor.The new wave of immunotherapies reflects what has been known for some time; that the answer to cancer is in you! But it is not that simple. Our immune system is complex and intricate & as science unravels its mysteries, we are developing new understandings of how the immune system can be captured & recruited in the laboratory; retrained and returned to you the patient!

As well on Navigating the Cancer Maze, Prof Khanna also discusses his research and clinical trial results on nasopharyngeal carcinoma, an aggressive throat cancer that has has shown response to immune therapies.
